I can use money as an additional deciding factor in my healthcare treatment plan. For instance, though I will be healthier sooner, is the value of a quick recovery worth $2,000? What could that $2,000 or $1,500 be better spent on to enhance my life when paired with a lower cost treatment plan? Etc. It is somewhat extreme, but I hope I am conveying my idea.
If you come to us, paying cash, we are glad to lay all that out and come up with a solution. I do it frequently.
The problem is that it is really a 3 party negotiation - you, practice, insurer. Only one party, due to power and law, has access to all the information needed to properly negotiate and that is the insurance company.
