They don't really work well on every lobe, as it were. Not everyone has a little pad at the bottom of their ears, that the back can be pressed above, to stop it from slipping.
I know I've tried to wear some like this, and had to screw them so tightly that I thought I was going to tear thru the existing piercing - and even THEN, they wanted to slip. It was painful, yo.
@Catnfiddle - if they have a loop at the top, where the screwback is attached, a jeweler can switch out the screwback for whatever you want... basically what I ended up doing. If not, I would find a jeweler in town who carries estate jewelry (sadly, I know none, other than Grandview Merch) - they're most likely to have a repair person that is up to the challenge.
